Form Instructions

In the performance of this site assessment, always wear appropriate PPE in compliance with your state regulations and your organization’s safety guidelines. This includes gloves, eye protection, fall protection, and electrical safety guidelines.
⚠️ If a system is energized and indicates a ground fault, the racking may also be electrically energized and pose a shock hazard.
Complete all sections of the form. If a section is not applicable, mark it as N/A.
Take photos of key components and attach them to the form when required.
Be objective and factual in your observations.
Report on things not found as well as those found. Do not leave a question in the report about the existence of a particular condition.
